The mission of Cumberland HealthNET is to provide access to holistic wraparound services that improve outcomes for individuals with health inequities.

- $25 can help a person with diabetes stay healthy and out of the emergency room.
- $35 can help a person have access to preventive care service such as lab work, access to a nurse to include care management and an initial wellness visit.
- $50 can help a person with hypertension have access to medication, regular checkup and lifestyle changes.
- $100 can help a person with multiple chronic illnesses obtain access to a stable medical home, equipment and less visits to the emergency room.
